At the beginning of every school year, it is important to update your child’s health information. This is done during online registration. If at any time during the school year your child has a new diagnosis of a health condition, please contact the school nurse to update the health information. Your child’s health information is private and protected by FERPA and HIPPA, any information will only be shared with appropriate staff in order to keep your child safe during school hours.

IMMUNIZATION REQUIREMENTS
INFLUENZA VACCINE
Every fall LDSD provides flu shots free of charge to any student signed up to receive one. Flu shots will be given on November 1st. All permission forms must be returned to the school by September 20th.

SCREENINGS
Hearing-Screenings are provided by SDSD in January to K, 1st, 3rd, 5th, 7th, and 11th.
Vision-Screenings are provided by our local Lions Club in the fall to K, 1st, 3rd, 5th, 7th, and 11th.
Dental-Delta Dental Bus will provide screenings and services to children that register. The Bus will be at Lead-Deadwood Elementary March 3rd-7th. There is no cost to the child or family. No insurance is needed. Registration/Consent must be completed and returned to the School Nurse no later than February 14, 2025.

At any time a parent may request that their student be screened for possible vision or hearing issues by the School Nurse.

Darcy Percy is a bachelor’s prepared registered nurse with previous clinical experience in orthopedics, ambulatory care, and home care. Darcy was a school nurse in Gillette, WY for 13 years before moving back to the Black Hills and has been Lead-Deadwoods District School Nurse since 2022. Darcy specializes in the care of children with complex medical needs and collaborates in the evaluation of children with educational barriers due to medical conditions.

Student Health Information
Medication Administration
Whenever possible, students are encouraged to receive medicine outside school hours. However, in those cases where medicine must be taken during the student’s time at school, the following procedures are to be followed:
All medication must be brought to school by the parent, guardian or a responsible designated adult.
All medications must be in the original container.
A signed copy of the Medication Consent Form must be on file.
Non-prescription medications should be in their original container, labeled with the student’s name.
High School students may carry and administer one dose of medication. The medication must be in its original container. A Medication Consent Form must be completed.
Emergency medications may be carried by a student with special written consent from the healthcare provider and the student must demonstrate to the nurse, the proper dosage and use of the medication.

Illness Protocol and Exclusion Criteria
Criteria for being excused from school:
Temperature of 100.4 F or greater.
Witnessed vomiting of more than a single episode (witnessed by staff).
Signs and symptoms of a communicable disease in need of medical evaluation/treatment.
School nurse discretion based on nursing assessment.
Nurse Excused/Exempt
The school nurse will call you and send home your student as nurse excused/exempt only when they meet exclusion criteria. This means they meet the above-listed criteria, and your student should be picked up as soon as possible. If your student does not meet the above requirement to go home, they will be sent back to class. Each situation may be different based on the student’s symptoms and school nurse assessment.
Students will be expected to be evaluated prior to calling home. If a student contacts their parent prior to evaluation and the parent chooses to pick the student up it will not be considered a nurse excused/exempt absence.
