FREE SPORTS PHYSICALS: The Monument Health Lead-Deadwood Clinic will offer free sports physicals during the months of June, July & August as long as an appointment has been made with one of the Clinic’s Advanced Practice Providers Please contact the clinic for details. You may call 717-6431 to make an appointment. A past history form has to be completed before your appointment. Packets (with the past history form) can be picked up at the Clinic, in the Business Office or printed from the link below.
